A first-time NBI clearance requires NBI Form No. 5 (the “Blue Form”) — the official Philippine government application form. You may obtain it yourself from a Philippine Consulate or Embassy, or, where we’re able, we can help arrange it for you as a courtesy in the Service & Delivery step below — free if picked up at our Toronto office (Ontario only), or shipped to you for a separate tracked-shipping fee.
Your fingerprints must be taken on Form No. 5 by a licensed (RCMP-accredited) fingerprinting provider in Canada. Fingerprinting is done in person and is a separate cost paid to the provider.
After you submit this form, our team will email you within 1 business day with the form and step-by-step instructions. Learn more: NBI Form No. 5 · Where to get fingerprinted.
Your NBI certificate can be lost or misdelivered in transit, and there is no insurance coverage. If it goes missing there is no recourse, and a reprint and redelivery cost extra. For tracked delivery we recommend Canada Post Xpresspost.
When your original reaches our Toronto office, we email you a link to book a pick-up time. Originals are held for 30 days from that email and are then securely shredded; a reprint after that costs extra, plus delivery. If you only need the digital copy, you do not have to collect the original.
We email you the tracking number once the carrier has your clearance. From then on, please follow the tracking and contact the carrier directly about the delivery itself, for example a missed delivery, a notice card, a parcel held for pick-up, or a delay. A claim for a lost or damaged parcel is normally made by the sender, which is us: tell us as soon as you notice a problem and we will make it for you. Any payment is limited to what the carrier pays under its own terms, which for a document is often little more than the postage, and we pass on to you what we receive.
Draw your signature below. This will be placed on your NBI application form.
